New Patients

As a new patient, you will be asked to sign the required authorization forms and provide us with a copy of your insurance card and previous medical records. You may request that your medical records be transferred from your previous healthcare provider by filling out the Authorization For Use Of or Disclosure of Health Information Formpdf file. While our service charges are based upon the severity of your illness as well as time spent with you, Heartland Family Medicine does its best to provide quality care while keeping your charges as low as possible.

Before your first visit, it will save time and smooth the process if you download the Patient Registration Formpdf file, print it, fill it out and bring it with you. We will also have forms available at the office if this is inconvenient.

Payment and Insurance

Heartland Family Medicine participates with many insurance programs including Blue Cross and Blue Shield of Nebraska, Midlands Choice, Medicare and Medicaid. Depending on your insurance provider, most insurance claims are sent directly to the insurance company, however, you are ultimately responsible for settling your account.

Statements are mailed monthly. For your convenience, Heartland Family Medicine accepts cash, checks, debit cards, Visa, MasterCard and Discover cards. Fees for service or insurance co-payments are required at the time of service.

Prescriptions

Your physician may authorize a prescription over the telephone, but please remember to ask for needed refills during office visits. Antibiotics and habit-forming medications require an office visit for a prescription. Refills may be requested by calling (308) 928-2103 during office hours.

Emergencies

If you have a minor injury, please call our office for assistance. For life threatening emergencies, call 911 or seek assistance in an emergency room.

Holidays Observed

Heartland Family Medicine will be closed on the following holidays: New Year’s Eve Afternoon, New Year’s Day, Good Friday Afternoon, Memorial Day, Independence Day, Labor Day, Thanksgiving, Friday Afternoon after Thanksgiving, Christmas Eve afternoon and Christmas.
